Dear Sir/Madam,
We are planning to arrange a training programme for a group of SEN students. Please send me the quotation about it. The details are as follows:
Date: Around 25 hours in the coming academic year (2021-2022)
Venue: Shun Lee Catholic Secondary School
Time: 1.5 hours (for lunch time or after school)
Students: 1-2 groups for 6 to 8 students (S1 to S5)
Service Description:
(1) Each student should receive an assessment of their SEN nature and ability.
(2) A comprehensive report on each student should be given to parents and teachers.
(3) Below is the list of the items included in the report:
a. Background information
b. Nature of the problem
c. Assessment
d. Detailed description of the assessment results
e. Recommendations
f. Progress of the student after training
g. Others
(4) The training should focus on their specific problem nature and attend to
their educational needs.
(5) Individual and group trainings will be required.
(6) Lesson observations by teachers and parents can be arranged (if
necessary).
(7) Parent interview will be arranged (if necessary) so that they can have a
better understanding of the students’ special educational needs.
Terms and conditions of the contract:
(1) At least one professional music therapist will come to our school.
(2) All notes, handouts and equipment will be provided by your organization.
(3) Our payment will be based on the total number of service hours and
the charge per hour.
(4) The quotation price should include labour supply, material supply
and mandatory provident fund, including sufficient insurance insured for
the contract, third party insurance and public responsibility insurance.
